I authorize the investigation of all statements contained in this application if I am considered for employment. I also authorize persons named as references to give the center any pertinent information regarding my employment, scholastic standing, etc.

I understand that if I am considered for employment, I must pass a criminal background check that will be processed before I am able to work in the classroom with children. 

I understand that any misrepresentation of the facts requested, unsatisfactory references, or failure to pass the physical examination would be sufficient cause for my dismissal from the center’s service if I have

been employed. 

I have never been convicted of any crime in any state for lascivious acts with a child, child neglect, child abuse, or had any drug charges in the last 5 years.
